Building system-wide theatre productivity that lasts

How a programme spanning 12 trusts increased theatre utilisation, reduced late starts and developed the local capability needed to sustain improvement.

The Challenge

The system had a major theatre productivity opportunity, but sustainable progress depended on stronger local capability.
Cheshire and Merseyside is one of the country’s largest integrated care systems, with acute and specialist hospitals operating across the region. A substantial RTT backlog and financial pressure made theatre efficiency a priority for every site. Several improvement programmes were already under way. The system recognised, however, that lasting performance would require better shared insight, more focused implementation and local teams equipped to lead improvement themselves.

What we changed

Productive Partners combined system-wide performance insight with targeted implementation and structured capability development.
Create shared performance visibility

Developed a system theatre productivity dashboard that allowed teams to identify consultant-level opportunities and compare performance across trusts.

Focus support on the biggest opportunities

Ran one-to-one Check, Challenge and Solution sessions with each trust, supported by OPCS-level benchmarking to identify priority procedures and implement HVLC and High Intensity lists.

Improve start-on-time performance

Launched a programme across six pilot trusts using clear communications, three-week resets and reviews of the steps leading up to the start of anaesthesia.

Build local capability

Delivered a six-month Theatre Academy for staff from across the system and created a Theatre Productivity Network to support continued shared learning.

The outcome

The programme improved performance across the system while creating the skills and infrastructure needed to continue the work locally.

Theatre utilisation increased by 9% across 12 trusts, with five trusts moving into the highest performance quartile in Model Hospital. The six start-on-time pilot sites reduced late starts by 40%, while average cases per session increased from 1.99 to 2.10.

Alongside these operational gains, 25 staff completed the Theatre Academy and were equipped to run effective productivity programmes within their own organisations. The new Theatre Productivity Network created an ongoing route for collaboration and shared improvement across the system.

Reflecting on the partnership
